This page explains, in plain English, what we collect, why, who sees it, how long we keep it and what you can ask us to do with it. The short version: we collect what you type into our case review form, we may share it with an Arizona law firm, as the consent wording you agree to says, and that firm may pay us.
Who we are. The Arizona Medical Malpractice Resource Center is operated by Wilder West Assets LLC. We are an information and referral service. We are not a law firm, a health care provider or an insurer. Privacy questions go to privacy@medicalmalpracticelawyersaz.com.
Lead disclosure: how this site makes money
We may send your request to an Arizona law firm, as the consent wording on the form says. A law firm that receives a request may pay us for it. We may also pass a request to a firm at no charge. This is the only way the site earns money. You never pay us.
What we collect when you use the form
- What you type: your name, phone number, email address, ZIP code, the approximate month and year of the care, and the short description if you choose to write one.
- A record of your consent: the fact that you checked the consent box, the exact consent wording shown to you, a version label for that wording, and a label for the type of request (medical malpractice).
- Technical details recorded automatically: the date and time you sent the form, the internet (IP) address it came from, the type of browser and device, and the address of the page the form was on.
We keep the consent record and technical details because we may have to show, later, that you agreed to be contacted.
What we ask you not to send
Please do not send Social Security numbers, medical record numbers, insurance numbers, images or your full medical history through the form or by email. We do not need them. If a lawyer takes up your request, they can ask for records in a safer way.
We are not a hospital, a doctor’s office or an insurer. Do not assume that health privacy laws that apply to them, such as HIPAA, apply to what you send us. We still treat what you tell us as sensitive.
How we use what you send
- To send your request to an Arizona law firm that may review your possible claim, as the consent wording says.
- To contact you about your request, using the phone number and email you gave.
- To keep a record that you agreed to be contacted.
- To count requests and keep the site working and secure.
A law firm that receives your request may pay us for it. That is the only time we are paid in connection with your information. We do not sell, rent or give what you send to anyone else, and we do not use it to build advertising profiles.
Who sees it
- An Arizona law firm. Your request comes to us first. We may then send it to a law firm that may review your possible claim. The consent wording you agree to before sending the form says this.
- Companies that run the site for us. Our web host stores the form entries, our email provider carries the notification message (a copy of your request is emailed to us), and our backup storage provider holds copies of the site database, which includes form entries. They handle the information only to provide those services.
- People we are legally required to answer. We may disclose information if a court order, subpoena or other legal process requires it.
Once a law firm has your request, that firm’s own privacy practices and its professional duties apply to what it holds.
Calls, emails and texts
By checking the consent box you agree that we, and a law firm that receives your request, may contact you by phone, text message and email about your request, at the number and email you gave. Message and data rates may apply. You can reply STOP to opt out of texts. You can withdraw your agreement at any time by telling the person who contacts you or by writing to privacy@medicalmalpracticelawyersaz.com.
How long we keep it
We keep form entries and consent records for five years from the date you sent the form. We keep them that long so we can show that a person agreed to be contacted if the question ever comes up.
Your choices
- Ask what we hold. Write to privacy@medicalmalpracticelawyersaz.com from the email address you used on the form.
- Ask us to correct or delete it. We will delete what you sent. We may keep the minimum record of your consent (what you agreed to, and when) for the rest of the five years, because we may need to show that you agreed to be contacted.
- Ask us to stop contacting you. Tell us once and we will stop.
If we have already sent your request to a law firm, you will need to ask that firm as well. If you ask us, we will tell you which firm it was.

Visiting the site without using the form
Like most websites, our server records basic details of each visit, such as the IP address, the pages requested and the time. We use these to keep the site secure and working. A security plugin on the site also logs sign-in attempts and blocked requests.
We currently use analytics and/or advertising cookies.
Security
We use reasonable measures to protect what you send, including an encrypted connection (HTTPS) and restricted access to form entries. No website can promise perfect security. That is one reason we ask you to keep sensitive numbers and records out of the form.
We are not lawyers. What you send to us may not be protected by attorney-client privilege. Keep it short and leave out anything you would not want others to see.
Children
The form is for adults. A parent or guardian may use it to ask about care their child received, so a request may include information about a child.
Where we operate
This site is about Arizona law and is meant for people in Arizona. We operate in the United States.
Changes to this policy
If we change this policy we will update the effective date at the top. If a change affects how we share information you already sent, we will not apply it to your information without asking you.
